Returning Student Information Form Instructions

Please read the following information before entering the Portal.

There are now two ways to access this form, which is due on September 15th, 2023.

Method 1, which is new for 2023-24, uses the PowerSchool Parent Portal and eliminates the need for the Snapcode. When you sign into the portal, a link to the Returning Student Information Form (2023-24) is listed in the left menu. Click on that and you will be taken to the form where you will enter the student’s birthday, bypassing the Snapcode entry. If you need to complete the form for additional students, return to the PowerSchool Portal after submitting the form to select the link for the next student.

Method 2 is the existing method and uses a 15 character Snapcode, which will be sent home in a letter on August 4th to each student enrolled in 2022-23.  If you do not receive our email notifications this information was sent home by postal mail and may take an additional few days. This Snapcode is only valid for the 2023-24 school year, so you do not need to keep this code once you submit the form.  Each parent/guardian will use the same login used to complete the 2022-23 Returning Student Information Form. If you moved into the district in 2022-23 or if you have a student who will be in Kindergarten in 2023-24, you will use the same login you created when you registered that student. If you do not remember the email address used when you created the account or no longer have access to it, and you need either your username or password changed, please contact the Registration Office at (724)981-8750, ext 6000. Once you enter the portal the student information we have on file will be filled in for you.  Please review this information carefully and make any corrections, where applicable.  If you have misplaced your Snapcode, please send an email to registration@hermitagesd.net with your student’s name and grade and we can email the code to you or resend the letter via postal mail or email.

If you have an address change to submit, please contact the Student Registration Office at (724) 981-8750 extension 6000.

Depending on the grade your student is entering, you may be required to provide an updated Physical Packet, Dental Form or Immunization Records.  If your student is entering one of those grades and needs to provide this information, you will be prompted to do so.  If you have already provided that information to the school nurse, you do not need to submit it again.  If you are unable to scan the necessary forms, please choose the option to send a copy to your school nurse on the first day of school.  Alternatively, if your students needs an updated physical or dental exam, you can elect to have your child seen by the school physician or dentist to meet those state requirements.

When you click on the link below, you will be asked to enter your 15 character Snapcode.  You will then be asked to enter your account username and password, which is the same account used last year for this form or when you registered your student.  Once signed in you will then be asked to enter the student’s birthday to confirm the Snapcode you entered.
